Educational Qualification starting from 10th standard; stating subjects, school/university, year of passing, percentage of marks, 10. Experience 11. Sponsored- / Non- sponsored (if sponsored - attach the certificate from employer), 12. Signature. Attach self-attested copies of proof of item No. 3, 6(if OBC/SC/ST), 9, 10 and 11.
